Referrals

If you are interested in referring someone to our services, please follow the steps below.

Who Can Be Referred?

We accept referrals for adults (18 years and older) who have learning disabilities or autism. Our services are designed to support individuals in achieving greater independence, improving life skills, and engaging in meaningful activities.

Eligibility Criteria

To ensure that our services are the best fit for the individual, we ask that referrals meet the following criteria:

• Age: 18 years or older.

• Diagnosis: Learning disability, autism spectrum disorder, or other associated conditions.

• Support Needs: Individuals should require support in daily living, social interaction, communication, or other areas related to their condition.

• Residential Status: Open to those living independently or in supported housing

How to Refer

Referrals can be made by professionals, caregivers, or the individuals themselves (with support).

To make a referral, please complete the following steps:

1. Download and Complete the Referral Form

Download the form here. Please provide as much information as possible to help us understand the individual’s needs, background, and goals. This includes medical history, current support arrangements, and any specific requirements.

2. Submit the Referral

You can submit the completed referral form via email: referrals@thewoodlandcentre-eltham.co.uk. Upon receipt, our team will review the referral form. If additional information is needed, we may contact you for further details.

If the referral meets our eligibility criteria, we will arrange an assessment to determine how best we can support the individual.

2. Assessment and Placement

Following the assessment, a tailored support plan will be created for the individual. If we can offer a place at our centre, we will arrange a start date and discuss the next steps.

What Happens After the Referral?

Once a referral is received and reviewed, we will:

• Contact you to confirm receipt and clarify any questions.

• Arrange an initial assessment meeting (either in-person or virtually) to understand the individual’s needs in more detail.

• Provide a tailored plan for how our centre can best support the individual.

• Keep you informed of the progress throughout the process.
